For the application for C.U.S. – C.U.S.I. membership you need:

For students: 1 proof of enrollment at the University of Florence for the current Academic Year.

Valid for registration purposes are: postal receipts for university fees; stamped university booklet (only for first-year students); certificate of enrollment at the University (the printout downloaded from the SOL service on the UNIFI website is also acceptable); declaration from the head of the specialization course

– medical certificate for non-competitive sports activity showing the wording “annual validity” (copies will only be accepted in the presence of the original document)

For University employees: University of Florence ID card and medical certificate for non-competitive sports activity showing the wording “annual validity”

For affiliated members (Val di Rose only): declaration from the affiliated institution or association and medical certificate for non-competitive sports activity showing the wording “annual validity”

FOR ONLINE REGISTRATION:

1. Upload the medical certificate for non-competitive or competitive sports activity in PDF format (PHOTOS WILL NOT BE ACCEPTED)
2. Fill out and submit the DECLARATION of the medical certificate;
3. Upload 1 proof of enrollment at the University of Florence for the current Academic Year: valid for registration purposes are: postal receipts for university fees; stamped university booklet (only for first-year students); certificate of enrollment at the University (the printout downloaded from the SOL service on the UNIFI website is also acceptable); declaration from the head of the specialization course. IF GRADUATING or SPECIALIZING upload the proof of enrollment at the University of Florence from the previous academic year.
4. Upload a photo of your face in JPEG format

**The BASE membership allows:
– registration for all paid courses;
– registration for tournaments organized by CUS Firenze (students and university employees only);
– booking of 5-a-side soccer, 7-a-side soccer, tennis and multi-purpose courts (volleyball and basketball) at CUS Firenze facilities.

**The OPEN membership allows:
– registration for all free courses (University Sports Center and Val di Rose facility);
– free access to the weight rooms at the University Sports Center;
– registration for courses through the website;
– registration for all paid courses;
– registration for tournaments organized by CUS Firenze (students and university employees only);
– access, during opening hours, to the Val di Rose sports facility in Sesto Fiorentino;
– booking of 5-a-side soccer, 7-a-side soccer, tennis and multi-purpose courts (volleyball and basketball) at CUS Firenze facilities.

**The UNIVERSITY SPORT CENTER membership allows:
– registration for all free courses (University Sports Center);
– free access to the weight rooms at the University Sports Center;
– registration for courses through the website;
– registration for all paid courses;
– registration for tournaments organized by CUS Firenze (students and university employees only);
– access, during opening hours, to University Sport Center in Firenze;
– booking of 5-a-side soccer, 7-a-side soccer, tennis and multi-purpose courts (volleyball and basketball) at CUS Firenze facilities.

**The VAL DI ROSE membership allows:
– registration for all free courses (Val di Rose facility);
– registration for courses (Val di Rose facility) through the website;
– registration for all paid courses;
– registration for tournaments organized by CUS Firenze (students and university employees only);
– access, during opening hours, to the Val di Rose sports facility in Sesto Fiorentino;
– booking of 5-a-side soccer, 7-a-side soccer, tennis and multi-purpose courts (volleyball and basketball) at CUS Firenze facilities.
